Medication Management

True Care’s staff is made up of highly trained, multicultural, multilingual Nurses who truly care about our clients.

What is Medication Management?

Medication management generally refers to organizing someone’s daily medications and overseeing their administration. True Care’s Nurses go beyond traditional medication management to provide the personal touch that characterizes True Care. We help you understand your medications, their effects, and how to safely maintain a consistent dosing schedule. In-person visits provide an opportunity to teach you about any new medications, discuss side effects, and mitigate concerns that may arise.

Our Nurses ensure that all unused medications are disposed of safely, and that clients are provided medications on schedule. They can also coordinate refills and liaise with your pharmacy to take the guesswork out of juggling multiple prescriptions.

Our Nurses are trained to assist with medication management. This means they help loved ones keep track of their medications and ensure they are taking the right dose, at the right time, for the right condition. They can help with medication-related problems such as confusion, depression, insomnia, muscle weakness, and falling.

At True Care, we assess each medication’s appropriateness, effectiveness, safety, cost, and compliance. The goal is to improve the client’s health outcomes. Our RN Case Managers work with the client, Physician, Caregiver(s), Pharmacists, Nurses, and other healthcare providers to accomplish this goal.

The RN Case Manager will continue to visit and communicate with the client, family, or Caregiver on an ongoing basis to assess the situation, monitor results, and recommend changes as needed. We continually adjust and adapt the plan as the client’s physical or mental condition evolves.

Nursing for Medication Management

If you or a loved one have been prescribed a number of different medications, keeping track of when and how to take them can be confusing and sometimes frustrating. True Care’s Nurses can help take the stress out of a complex medication regimen.

    Katherine A

“I love this home care. My grandma has been with this agency for the past 7 years and she never had a problem/ bad experience. They always prompt on substitutes if needed or any updates that are happening at the agency. I myself called them a couple of times to clear out a few questions for my grandma and the front desk ladies were so friendly and helpful! Very satisfied!”

    V. S.

True Care really does care about their clients. It’s taken a little while to get to the aid that I currently have but she’s irreplaceable and hard working. Everyone I’ve had the pleasure of speaking to is professional and kind. I’m very thankful that we found this agency because there are a lot of Home Care agencies out there but very few that really care, consider their clients and ultimately want what’s best for them.
