Home Care Services

Regardless of age or stage, be it due to short-term health issues or chronic conditions, healing at home is often the best-case scenario for most people. That’s where home care comes in.

True Care provides professional care and support through professionally trained Caregivers (certified Home Health Aides), in your home or community-based setting to allow for maximum independence and the very best quality of life.

Nurse Services

When the care you need extends beyond the scope of what a Caregiver can safely provide, True Care is standing by with nurse services. Our nurses come to your home so you can remain where you are most at-ease, providing the ideal alternative to frequent visits to the doctor, long-term care, or nursing home facilities.

True Care believes that the best healing can happen when a client is home among familiar faces and surroundings. When you or your loved one is recovering from an illness, procedure, treatment, or is experiencing a decline in health, our nurse services offer peace of mind and help maintain quality of life.

CDPAP Services

CDPAP is the ultimate intersection of home and care. It is a Medicaid-sponsored New York State program that pays for a trusted loved one to provide your in-home care. This program allows you to choose a family member or close friend to provide home care services and have them get paid for it. You will direct your own care including hiring and training your chosen caregiver. And with True Care guiding every step of the authorization process, you can rest assured the logistics are taken care of. The care you need, with the caregiver you choose.

Private Client Services

True Care’s Private Client Home Care Services offers a concierge suite of white-glove services that empowers our clients with the individualized care they deserve. Our Private Pay programs provide all the benefits of home care on an out-of-pocket basis.

    Katherine A

“I love this home care. My grandma has been with this agency for the past 7 years and she never had a problem/ bad experience. They always prompt on substitutes if needed or any updates that are happening at the agency. I myself called them a couple of times to clear out a few questions for my grandma and the front desk ladies were so friendly and helpful! Very satisfied!”

    V. S.

True Care really does care about their clients. It’s taken a little while to get to the aid that I currently have but she’s irreplaceable and hard working. Everyone I’ve had the pleasure of speaking to is professional and kind. I’m very thankful that we found this agency because there are a lot of Home Care agencies out there but very few that really care, consider their clients and ultimately want what’s best for them.

    Frequently Asked Questions

What are the main services that True Care offers?

Our main services include Home Health Aide, Private Client Care, Home Infusion Therapy, Memory Care, Private Duty Nursing, and Hospice & Palliative Care.

What is the difference between home care and home health care?

“Home care” is a term used to describe any type of non-medical assistance someone receives in their home due to an injury or illness. This assistance is provided by a professional caregiver. These services include cleaning, cooking and other tasks for seniors or others who cannot perform these tasks on their own.

“Home health care” is the treatment of illness, injury, disability, or disease through nursing, medical, and other health-related services. These services are provided in the comfort of the patient’s own home and must be performed by a licensed healthcare professional. Home health care requires a written order from a doctor, and is usually performed by a nurse, physical therapist or occupational therapist who comes to the house to provide treatment.

While they may sound similar, “home care” and “home health care” are actually not the same services. Knowing the difference between home care and home health care can be a huge help when it comes to making an informed decision about how to help your loved one at home
