“Home care” describes any type of medical or non-medical in-home assistance a person receives to help them heal from illness, injury, or another major health event. This assistance is provided by a professional Caregiver / Home Health Aide (HHA), a Registered Nurse (RN), or Licensed Practical Nurse (LPN).
Through True Care, families can work with one of our experienced Caregivers, or with a loved one of their choosing through the New York State CDPAP program.
We assist clients with the application process to ensure continuity of care and satisfaction with the Caregiver. Our Caregivers are selected not only for their caregiving skills but for their character and warmth to deliver our signature home care experience.
Home care can be life-changing for older adults who want to stay in their own homes and live independently, but need support to do so. It can also provide peace of mind to families that are worried about their loved one’s safety, but cannot take care of them on a regular basis.

A large percentage of our clients are covered under Managed Long-Term Care health insurance plans through Medicare and/or Medicaid, however private pay options are also available. We work with clients of all financial backgrounds to coordinate the best possible care for you or your loved one.

The first step is to sign up with a Managed Long-Term Care (MLTC) insurance plan and be sure to register with a plan that covers home care. All the enrollment documentation and necessary forms will be sent to you by a True Care Intake Representative.

NYIA Assessment and Determination
Your Intake Representative will assist you with scheduling your New York Independence Assessment (NYIA). A New York State Nurse will meet with you either at home or online to help determine whether or not you qualify for home care. The NYIA Nurse will determine the type of services and the number of hours that you are approved for.

MLTC Application Approval
During this step, your MLTC insurer will authorize your hours and schedule you to begin receiving services.

Caregiver Selection
We take the time to get to know your preferences and source Caregivers (also known as Home Health Aides or HHAs) that match your requirements for care. We have a diverse team of Caregivers that will be able to cater for your specific needs. We will continue searching to find the right fit for you until you are satisfied and feel you are in good hands with your trained, experienced, and certified Caregiver.

True Care Nurse Visit
A True Care Registered Nurse (RN) will visit your home at the start of your care to determine the role and responsibilities of the Caregiver, depending on your needs. Our Nurse will assess your home care needs, learn about your preferences, and develop the right Plan of Care for you.

Wellness Call
Throughout the first week of service, our Quality Assurance team will conduct a Wellness Call to ensure your needs are being met. We are available 24/7 to take your calls should you have any additional questions. Your feedback is of great value to ensure we continue to provide the highest level of care.

Scheduling Coordinator
Once you are satisfied with your Caregiver, you will be assigned a designated Scheduling Coordinator who will be your point-of-contact for all your needs and questions from that point onward.

When should I get a Caregiver (HHA) for my loved one?

True Care can help guide you every step of the way from finding the right caregiver to ensuring your loved one’s well-being is safely taken care of.What if I don’t like my Caregiver?
If you are not happy with the services provided by your assigned HHA, True Care would be happy to reassign you a caregiver more suited for your needs and desires. You can also try our CDPAP program, which frees you to designate a friend or a loved one as your dedicated home care aide.
What certifications do your caregivers have?
Our team of home care professionals is comprised of certified HHAs, LPNs, and RNs. We require more than standardized industry training, and our caregivers are chosen for their character and warm communication to deliver our signature in-home care experience.
