Our home care services are covered under Medicare or Medicaid Managed Long-Term Care (MLTC) plans at no cost to you. MLTC can help you if you have a disability or chronic illness and want to remain home. MLTC plans are provided through managed long-term care plans approved by the NYS Department of Health. We can assist you with the Medicaid application process. We also accept private pay.

Registration

Sign up with an MLTC insurance plan. We can help you with the application process.

NYIA Assessment and Determination

Your Intake Representative will assist you with the New York Independence Assessment (NYIA). A NYS Nurse will meet with you to help determine whether you qualify for home care, and the services and hours you need.

MLTC Application Approval

Your MLTC insurer will authorize your hours. Now you can start receiving services.

Caregiver Selection

We provide a caregiver who can meet your unique needs.

True Care Nurse Visit

One of our Registered Nurses (RN) will visit you at home to determine the caregiver’s role and responsibilities. The RN will assess your home care needs, learn about your preferences, and develop the right care plan.

Wellness Call

In the first week of service, our Quality Assurance team will conduct a wellness call to ensure your needs are met.

Scheduling Coordinator

Once you are satisfied with your caregiver, you will be assigned a Scheduling Coordinator who will be your point of contact for all your needs and questions.
