What Do Care Managers Do?
Care Managers can help families and their loved ones cope with complex issues and help them with decisions connected to illness and aging. This includes assistance in all aspects of long-term care. A Care Manager will often begin working with a family when an older adult has mental health issues, is experiencing mental decline, or has health issues that are too complicated to be managed alone. When families need assistance to navigate the healthcare system and make decisions, Care Managers are by your side to assist in bringing the client’s plan of care into focus.
Their goal is to improve client health and well-being, aiming to individualize health care to meet each client’s specific needs.
How Does Care Management Work?
First, Care Managers will conduct an in-person assessment. This assessment allows them to gauge their client’s needs, enabling them to create a personalized plan and select services that are specifically tailored to meet those needs.
Care plans include the results of the assessment, recommendations, and referrals for local community resources. Care Managers can also monitor client needs and capabilities as time goes on. They do this to ensure that the quality of care is up to par and evolves alongside the client.
