MANAGED LONG TERM CARE (MLTC) PLAN OR SELF-PAY

Sign up with an MLTC plan, a type of health insurance that covers home care services. We can help you with the application.

NURSE ASSESSMENT

A NYS Nurse reviews your home care needs, and determines services and hours required. After approval from your MLTC insurer, you can start receiving care.

CAREGIVER MATCHING

You are matched with a True Care Certified Caregiver. One of our Registered Nurses (RNs) outlines your Caregiver's duties based on your specific needs.

True Care will help you every step of the way.

We provide dedicated professionals to guide you through the enrollment process and personalized Care Plans.

A caregiver is an option when you see your loved one struggling to perform everyday activities, such as bathing, dressing, preparing meals, going up and down the stairs, or doing household tasks.

Our caregivers undergo extensive home care training.

They speak 13 different languages, including English, Spanish, Russian, Mandarin, Cantonese, Bengali, French, Haitian Creole, Arabic, and Vietnamese.

Caregivers provide a companion to help you complete everyday tasks, including bathing, dressing and grooming, light housekeeping, meal preparation, and laundry. In addition to accompanying, you to grocery shopping, family events, and social activities, caregivers can travel with you to medical appointments.

We accept both Medicaid and Self-Pay for our Home Care Services.

To receive home care, you must be enrolled in Medicaid. We can help you enroll if you don’t have Medicaid. Our Support Services are covered by Medicare.
