The True Care approach allows you to receive the necessary support while remaining in the place you cherish most. Our home care team consists of Home Health Aides (HHAs), Registered Nurses (RNs), and Licensed Practical Nurses (LPNs). Home care can be life-changing for older adults and people with disabilities who need additional support.
Our Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) come to your door. Nurses help you with Medical Transportation, Wound Care, Infusion Therapy, Care Management, and Medication Administration and Management, offering the ideal alternative to long-term care institutions or nursing homes. People with injuries, chronic illnesses, or disabilities can benefit greatly from this service.
True Care’s private client services empower clients with care services at home. The private pay program provides all the benefits of home care on an out-of-pocket basis.
If you are not eligible for a Medicaid or Medicare LTC and need in-home care, consider our private client services.
The Medicaid-sponsored Consumer-Directed Personal Assistance (CDPAP) pays a trusted loved one to provide your in-home care. This program allows Medicaid recipients needing home care to direct their own care, including selecting and managing the family member or close friend who will care for them and get paid. True Care will guide you through every step of the authorization process, so you can receive the care you need with the home caregiver you choose. People who have a chronic illness or disability that requires assistance with daily activities can benefit greatly from CDPAP.
We provide support like Medicaid Eligibility Assistance, Home-Based Medical Care, Remote Patient Monitoring (RPM), Medical Transportation, and Chronic Care Management (CCM). The Community Health Initiatives (CHI) team creates custom care plans, arranges transportation, and coordinates medical services. If you are 65 or older and have Medicare, you may be eligible for medical equipment, prescription delivery, and home care services — at no additional cost.
