If you’re enrolled in Medicare, True Care’s CHI Services can help you with things like free transportation, safe housing, food assistance, social connections—all of which can make a big difference in your overall health. We'll connect you with local resources, schedule your medical appointments, explain your health conditions in plain language, and support you in making confident choices about your care. We’ll also help you learn how to speak up for your needs and navigate the healthcare system.

If you’re enrolled in Medicare, True Care’s CHI Services can help you with things like free transportation, safe housing, food assistance, social connections—all of which can make a big difference in your overall health. We'll connect you with local resources, schedule your medical appointments, explain your health conditions in plain language, and support you in making confident choices about your care. We’ll also help you learn how to speak up for your needs and navigate the healthcare system.
You may be eligible for medical equipment, prescription delivery, and Home Care Services at no additional cost.
Despite being eligible, many individuals do not fully use Medicare Services due to a lack of awareness. According to the Centers for Medicare & Medicaid Services, 25% of U.S. Adults have Medicare, but many miss out on essential benefits because they do not understand how to access them.
