What is Memory Care?

So much of who we are is made up of our memories. Reminiscing about good times, reflecting on our youth, and recalling events are essential to not only social but also our executive functioning.

Memory loss can be devastating to individuals and families, however, there is hope. At True Care, we bring best-in-class memory care into our clients’ homes with our True Bridge Memory Care program.

Memory care provides intensive, specialized care for people with memory impairments. True Care’s True Bridge program is designed to empower, encourage, and engage our clients in order to provide a greater level of care for older adults living with Alzheimer’s, dementia, and other forms of memory loss.

Memory care begins with a comprehensive clinical assessment to determine the capabilities and strengths of the client. This assessment provides the family and care team with the information, support, and necessary training to best meet the client’s needs. Our memory care Nurses are trained to take a full history, which includes clients’ preferences, cultural background, abilities, and health goals. This information is then used to craft a custom care plan that honors, supports, and engages the client in the most dignified way to enhance both safety and purposeful living.

How does Memory Care work?

Memory care programs like True Bridge are designed to provide a safe and supportive environment for individuals with memory loss, while also helping to manage their symptoms and improve their quality of life. Memory care programs typically offer a range of services, including assistance with the Activities of Daily Living, medication reminders and management,

social and recreational activities, and specialized therapies designed to appeal to the unique strengths and interests of the client. Select Caregivers receive additional training in order to be able to provide the highly sensitive, specialized services that memory care requires.

Our True Bridge program starts with an Engagement Assessment, which identifies each client's strengths, interests, and abilities. We provide the tools to enrich the client's days and create connections with family and caregivers.

Families are encouraged to participate and are an important part of the client's support circle. Together, we can face the challenges of memory loss.

True Bridge provides intensive and ongoing training to prepare our Nurses and Caregivers for the demands of caring for those with memory impairments. We also work with families to help them learn the best ways to care for and interact with their loved one who is experiencing memory loss.
