To be eligible for Our Family Caregiver Plan, you must:
– Be a Medicaid recipient.
– Require home care services.
– Be able to self-direct your own care or have a designated representative to do so.

What documents are needed for the application?
You will need:
– Proof of Medicaid eligibility.
– A physician’s order for services.
– Identification documents for the caregiver.
– Any additional documents required by True Care or the Medicaid Managed
Care Plan.
Who can be a caregiver under our Family Caregiver Plan?
A caregiver can be:
– A family member (excluding parents of minors and spouses).
– A friend.
– Any person chosen by the consumer who meets the program’s
requirements.
How many hours of care can I receive through our Family Caregiver Plan?
The number of care hours you can receive is determined by an assessment of your needs conducted by a Medicaid Managed Care Plan or a local social services office.
What services does our Family Caregiver Plan cover?
Our Family Caregiver Plan covers a wide range of services, including but not limited to:
– Personal care (bathing, dressing, grooming).
– Assistance with mobility.
